Adult Psychiatric Rehabilitation Program

Adult Psychiatric Rehabilitation Program (PRP) at HACS is designed for adults who are 18 years of age and older. This program is designed to teach or restore skills necessary for recovery from mental illness. This program offers group and individual services to participants. Many patients report improved symptom management and better life skills after participation in the PRP Program.

HACS provides adults with structured individual and group sessions, designed to develop and enhance independent living skills and the management of mental illness symptoms. The goal is to assist individuals with the transition from a long period of hospitalization to successfully re-enter the community, and to assist those individuals already in the community with maintaining and enhancing their skills.


ADULT SERVICES WILL CONSIST OF:

Self-care skills

  • Personal Hygiene, grooming, nutrition and self-administration of medication

Social skills

  • Community Integration Activities, Developing Natural Supports and Developing linkages with community activities
  • Independent living skills, housing stability, community awareness, mobility and transportation skills, wellness self-management and money management
  • Accessing available entitlements and resources, obtain and retain employment, activities that support cultural interests, health promotion activities (proper nutrition and exercise) and promotion of individual Wellness, self-management and recovery

Supported Housing Services

  • Collaborate with CSA to access affordable housing and develop financial resources
  • Housing needs assessment (housing needs, rehabilitation skills needed to obtain housing of choice, evaluate financial resources, develop basic living skills and supports to keep housing, available natural supports, needed and desired community services, private and public housing assistance).

Admission Criteria For PRP Services

  • The individual must be at least five (5) years old.
  • The individual must have active Medical Assistance or agree to private pay terms.
  • The individual must have an Axis I diagnosis of severe or chronic mental illness that can be verified through a therapist (LCPC or LCSW-C) and / or a psychiatrist
  • The individual must have a serious impairment in his/her ability to function independently in the community.

Referral Process For PRP Services

  • The referral must be completed by the treating Psychiatrist or Licensed Therapist (LCPC or LCSW-C).
    Once the referral is received, HACS staff will check the medical assistance number to ensure that it is active and then schedule a face-to-face interview with the potential participant.

    After the interview, the staff will submit all information to the State to receive authorization to provide services.

    Once the authorization is obtained, HACS staff will contact the referral source and client to inform them of the program acceptance and set up a start date and transportation.
